Third Party Complaints

If you are having any problems or concerns about third party dental insurers, PDA wants to hear from you. Please complete the Third Party Complaint form. This form gives dentists and dental staff the opportunity to provide PDA and ADA with basic information regarding payer issues and complaints. The information received will be used to keep a close watch on carrier trends and problems.
